Fall Protection Requirements in Construction: What OSHA Mandates at 6 Feet and What Your Daily Shift Reports Must Document

The 6-foot rule under OSHA Subpart M

OSHA's fall protection standard for construction is found in 29 CFR 1926 Subpart M (Sections 1926.500 through 1926.503). The core requirement is straightforward: fall protection is required when a construction worker is on a walking or working surface with an unprotected side or edge that is 6 feet (1.8 meters) or more above a lower level.

Where the 6-foot rule applies

The 6-foot trigger applies broadly across construction activities, including work near:

  • Unprotected sides and edges of floors, roofs, ramps, and runways

  • Leading edges during construction

  • Hoist areas and holes (including skylights)

  • Formwork and reinforcing steel

  • Excavation edges

  • Wall openings

  • Dangerous equipment at any height where a fall could result in injury

The standard also applies at heights below 6 feet when workers are near dangerous equipment, such as machinery with open drive belts or pulleys, where a fall of any distance could cause serious injury.

What Subpart M requires

Section 1926.501 establishes when fall protection is required. Section 1926.502 sets the criteria for approved fall protection systems. Section 1926.503 establishes training requirements. Most citations come from Section 1926.501 (failure to provide fall protection) and Section 1926.503 (failure to train and document training).

The three approved fall protection systems

OSHA recognizes three primary fall protection systems for construction. The contractor must select the system appropriate for the work being performed and the conditions on site.

Guardrail systems

Guardrail systems are passive fall protection, meaning they protect workers without requiring the worker to do anything. A compliant guardrail system includes a top rail at 42 inches (plus or minus 3 inches), a midrail, and must withstand a 200-pound force applied in any outward or downward direction at the top rail.

Safety net systems

Safety net systems catch workers after a fall and must be installed as close to the working level as practicable, but no more than 30 feet below. Nets must extend outward from the edge a specified distance depending on the height. Safety nets are common in bridge and structural steel work where guardrails and harnesses may be impractical.

Personal fall arrest systems

Personal fall arrest systems are the most commonly used method in specialty trade construction, particularly for workers on structural steel, roofing, and elevated mechanical installations.

A personal fall arrest system (PFAS) consists of a full-body harness, a lanyard or self-retracting lifeline, and an anchorage point. The system must be capable of arresting a fall within specified limits to prevent the worker from contacting any lower level. Anchorage points must support at least 5,000 pounds per worker attached, or be designed and installed under the supervision of a qualified person with a safety factor of at least two.

Personal fall arrest systems require individual training and fit verification for each worker. A harness hanging on a nail in the trailer is not compliance. Every worker using a PFAS must be trained on proper donning, connection, and self-rescue, and the training must be documented in writing.

Selecting the right system for the task

The choice between guardrails, nets, and personal fall arrest depends on the work being performed, the duration of exposure, and the feasibility of installation. Guardrails are preferred for long-duration work areas with defined edges, such as floor openings and roof perimeters. Personal fall arrest systems are better suited for mobile work at height, where the work location changes frequently.

On many specialty trade projects, multiple systems are used simultaneously. A crew installing HVAC equipment on a roof may work behind guardrails at the perimeter while wearing harnesses tied off to anchorages near openings. Documenting which system is in use at each location and for which workers is what the ​daily shift report must capture.

What daily shift reports must document for fall protection

OSHA does not prescribe a specific daily documentation form for fall protection, but the practical reality is clear: contractors who cannot demonstrate that fall protection was in place, that workers were trained, and that conditions were assessed on each shift face significantly higher citation and penalty exposure.

Elements to capture on every shift

A ​daily shift report that includes fall protection documentation should capture the following for each elevated work area:

  • Fall hazards identified and the protection system in use (guardrail, net, or PFAS)

  • Workers assigned to elevated work and confirmation that each has current fall protection training

  • Condition of fall protection equipment (harnesses inspected, guardrails intact, nets properly installed)

  • Any changes to work conditions that introduced new fall hazards during the shift

  • Near-miss events or corrective actions taken

Training documentation requirements

Training must be documented with a written certification record that includes the worker's name, the date of training, and the signature of the trainer or competent person. Retraining is required when there is reason to believe a worker does not have the understanding or skill needed, when changes in the workplace render previous training obsolete, or when changes in fall protection systems require updated instruction.

Fall protection beyond the 6-foot baseline

Specialty contractors working in specific conditions face additional or modified fall protection requirements under other OSHA subparts.

Roof work

Low-slope roofs (4 in 12 or less) allow additional system options, including warning line systems combined with other protection. On low-slope roofs 50 feet wide or less, a safety monitoring system alone may be permitted. Steep roofs (greater than 4 in 12) require guardrails, safety nets, or personal fall arrest systems, with no exception for monitoring alone.

Steel erection

Steel erection work is governed by Subpart R (29 CFR 1926.760), which sets a higher trigger of 15 feet for connectors and 30 feet for deckers under certain conditions, with specific requirements for controlled decking zones.

Scaffolds

Scaffold fall protection is governed by Subpart L (29 CFR 1926.451), which requires fall protection at 10 feet for scaffold workers, with system requirements specific to scaffold type.
